Compensation : $40.00-$57.64 (DOE).
Sign-on Bonus : $2,000 paid after 90 days of employment.
Location : Central Oregon.
Schedule : Full time, Monday-Friday 8 : 00 am-5 : 00 pm.
Travel : Local / regional travel by personal vehicle during the working day, centralized from the Bend office. Mileage reimbursement is provided.
What You Get To Do : The Support Registered Nurse plans, coordinates, and evaluates home health services. They provide nursing care focusing on community health education and utilize organizational and community resources to meet the needs of patients and families at home.
- Complete an initial comprehensive (if HH includes OASIS) and accurate assessment of the patient and family to determine home health needs. Provide a complete physical assessment and history of previous illness (es) and use the health assessment data to determine the appropriate diagnosis and need for other disciplines and resources.
- Provide nursing procedures and treatments per plan of care. Administer medications and treatments as prescribed by the physician.
- Complete and update the ongoing assessments and care plan as the patients status changes.
- Provide health care instructions and education to the patient and / or family, per the assessment plan and the patients caregivers ability to learn. Adapt teaching styles to meet the patients and familys needs.
- Coordinate patient care with the patients case manager. Provide verbal or email updates as necessary.
- Communicates with the physician regarding the patients needs, reports, and changes in the patients condition; obtains / receives physicians orders as required.
- Participate in on-call duties as defined by the on-call policy.
