What are the responsibilities and job description for the Community Health Nurse - LPN position at Pascua Yaqui Tribe?
Duties of a Community Health Nurse LPN: provide direct patient care to include COVID related duties, conducting patient assessments, on-going case management, researching and locating additional resources, performing community outreach and education and acting as an advocate for appropriate patient care. The incumbent may perform duties in a variety of setting, in-home care, clinics, testing sites and immunization clinics

Special Requirements:
- Assists with COVID testing, administering COVID vaccinations and documenting all encounters.
- Performs nursing tasks; provide patient care through home visits or clinical visits including: vital signs, nursing assessments (in collaboration with a RN), point of care testing, administering vaccinations and/or other injectable medications, medication management and education, development and implementation of a care plan and evaluating the need for supplies or ongoing patient education in addition to documenting in E.H.R.
- Assists with and coordinates educational classes related to community health.
- Identifies areas of concern and refers for further services to other departments.
- Act as an advocate for patient care during escorts to appointments and explaining diagnosis, treatment plans and follows up with the client.
- Monitor and track health care needs of assigned patients; works with other medical providers to coordinate proper and effective care.
- Coordinate referrals and follow up services with other Tribal departments; responds to patient and staff inquiries.
- Provides education for community members, promotes family health, helps prevent disease, and maintains health regimens.
- Document all encounters into E.H.R. system.
- Follows operating procedures and policies.
- Perform other duties of a similar nature or level as requested by supervisor or director.

AND
- Experience as a nurse in a public health setting preferred;
- Must possess and maintain a valid Arizona Driver?s License;
Special Requirements:
- Current Arizona State License as a Practical Nurse (LPN); failure to maintain current license will result in termination;
- Current TB Test required;
- Current First Aid and CPR Certification;
- Must possess and maintain a valid Arizona Driver's License;
- Must have a current Level 1 Arizona Clearance Card; failure to maintain a current Level 1 Clearance Card will result in termination.
