GENERAL DESCRIPTION OF POSITION
Responsible for initiation and completion of the necessary procedural steps for establishment and implementation of direct provision of all supported living services for individuals assigned to Medicaid Waiver services. Responsible for coordination, monitoring an individual's service plan, services delivered, and progress; responsible for hiring, training, supervision/discipline, payroll, evaluation of Supported Living staff.
MONDAY-FRIDAY, 8:00AM-4:30PM
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Conducts a preliminary assessment for new referrals.
- Assists with developing a plan of service, participates in continued stay reviews and personal future plans.
- Determines direct care service delivery staffing needs and arranges for the delivery of services through conducting interviews, hiring, supervising, producing payroll payments, evaluating, and discipline of Supported Living staff.
- Participates in team meetings with family members, professionals, and care coordinators to implement procedures and programs that ensure regulation compliance.
- Coordinates schedules for both waiver and generic services for consumer that includes staffing of all alternative living services.
- Performs Medicaid billing payroll functions for Supported Living staff on a weekly basis and assures the integrity of that billing by approving MITC time, attendance and time cards, reviews and approves direct care travel, and documents work performed.
- Monitors all in-home services on a regular basis that includes ensuring Supported Living staff compliance with completion of progress notes, required paperwork, code of ethics, and meeting agency requirements for initial and continued employment.
- Ensures direct care staff is compliant in following positive behavior plans and completing reports as required.
- Completes in a timely and accurate manner annual, quarterly, and monthly reports and various other reports as determined by Supervisor.
- Provides training to new Supported Living staff in general procedures and individual specific training.
- Reviews Supported Living staff progress notes, inputs data into consumer workbook monthly, and creates quarterly reports that focus on recipient's progress in meeting goals and objectives.
- Counsels recipients and families regarding staffing issues, scheduling, and other issues under their scope of responsibilities, and is on call to assist with staffing emergencies.
- Responsible for monitoring client medication regimens, ensures staff is at all times aware of medications being used; monitors consumption of medications on at least a monthly basis; ensures screenings are performed at a frequency determined by the prescribing physician, and any additional requirements as determined by DDS rules and regulations.
- Prepares appropriate paperwork for new plans, continued stay reviews and revisions in accordance with DDS standards.
- Serves as the individual's advocate in assuring access to a wide range of services. Documents services that are needed by or recommended for the individual but do not exist within the individual's local community, and works to develop the availability of these services.
- Assures that all personal rights of the individual are maintained, including freedom of provider choice, civil, and legal rights.
- Perform any other related duties as required or assigned.
EDUCATION AND EXPERIENCE
High school or GED, plus 2 years related experience and/or training, and 7 to 11 months related management experience, or equivalent combination of education and experience.
REQUIRED CERTIFICATES, LICENSES, REGISTRATIONS
Must possess Arkansas drivers license, have current liability auto insurance, and be physically able to drive in various locations in Arkansas.
