We are seeking dedicated and compassionate Rehabilitation Specialists to join our team. In this role, you will work closely with individuals with a variety of physical, mental, emotional and behavioral issues. You will be providing support and guidance to enhance their quality of life. Your expertise in behavior management and direct support will be essential in helping clients achieve their personal goals and improve their daily living skills.
Responsibilities
- Develop and implement individualized rehabilitation plans tailored to each client's needs.
- Provide direct support to individuals in various settings, including home and community care.
- Be willing to be flexible in your scheduling to meet the needs of families.
- Assist clients with Activities of Daily Living (ADLs) to promote independence.
- Utilize behavior management techniques to address challenging behaviors effectively.
- Collect and analyze data on client progress to inform treatment plans.
- Collaborate with interdisciplinary teams, including healthcare professionals and family members, to ensure comprehensive care.
- Drive clients to appointments or community activities as needed.
- Maintain accurate records and documentation of client interactions and progress.
- Potentially work early mornings/evenings depending on the client needs
Experience
- MUST HAVE A BACHELOR DEGREE IN A HEALTH SCIENCE FIELD
- CPR/FIRST AID certification(Pathway can provide the training)
- Be able to pass a background check
- Experience working with individuals with behavioral, mental, emotional and physical concerns is preferred.
- Background in behavioral health or related fields is a plus.
- Familiarity with data collection methods and documentation practices.
- Skills in childcare or experience in direct support roles are beneficial.
- Strong communication skills and the ability to build rapport with clients and their families.
- A valid driver's license is required for transportation responsibilities.
- Ability to provide consistency and stability to clients is a major plus
- Being able to communicate needs to staff and supervisors

Pay: $18.00 - $20.00 per hour
Expected hours: No less than 15 per week
