What are the responsibilities and job description for the Community Based Rehabilitation Specialist/Case Manager position at Portneuf Valley Family Center,Inc.?
Portneuf Valley Family Center Inc. has a current employment opportunity. We are looking to hire for Community Based Rehabilitation Services (CBRS), and Case Management services. These services will be provided to children, adolescents, and adults who struggle with mental health concerns. This position is available in our Pocatello and Soda Springs offices.
We are a full-service mental health clinic working with a team of clinical professionals (Physician’s Assistant, Psychiatric Mental Health Nurse Practitioner, Therapists, CBRS, and Case Managers).
Applicants must have a Bachelor Degree in behavioral sciences, education, or related field. Bachelor level social workers that are not completing a master of social work program must have their LSW license.
CBRS is an in-home and community-based service where staff work one-on-one with
consumers to teach them life skills and assist them in working towards
recovery and resiliency in their own environment. Case Management
Services assist and link consumers to access services and needs in their
community. Hours are flexible with the ability to set their own
schedules. This position will be part-time to start with the potential to move full time when availability allows.
Job Types: Full-time, Part-time
Pay: $18.00 - $20.00 per hour
Expected hours: 20 – 30 per week
