What are the responsibilities and job description for the Certified Peer Support Specialist position at Pathway Homes, Inc?
Pathway Homes is a private, nonprofit striving to end homelessness by providing housing and services for adults with mental illness and other co-occurring disorders in Northern Virginia. We are seeking a Peer Support Specialist to join out Assertive Community Treatment (ACT) Team in the Reston/Herndon areas of Fairfax County.
An ACT program is an interdisciplinary team that provides wrap around services 7 days per week to individuals in their homes and in the community. The goal of program is to decrease the utilization of ERs, in-patient hospitalizations and involvement with the legal system, by supporting individuals in developing effective healthy living skills.
The PEER SUPPORT SPECIALIST:
- Uses personal experience to develop meaningful and trusting relationships with individuals, including those not engaged in services, and provides targeted outreach and support.
- Acts as a mentor and role model.
- Supports individuals in expanding their recovery support network.
- Assists individuals with basic daily living tasks such as shopping for food and other needed items, use of public transportation, etc.
- Engages individuals in social, recreational, and leisure activities in the community.
- Supports individuals in their recovery journey.
- rotating on-call
Required Qualifications: High School Diploma, lived experience of mental illness and/or substance use disorder, documented peer support or mental health services experience. Must have a valid drivers license and reliable vehicle. Must be able to pass a driving record and criminal background check.
Available Schedule:
- Tues - Wed 11am - 7:30pm & Sat 11a - 3pm
