Job Summary
Responsible for people supported in all aspects of community employment under the general direction of the Program Services Director.
Specific Responsibilities
- Assists person supported with pursuing community employment, applying for community employment, and interviewing for community employment.
- Provides transportation for person supported in all aspects of community employment.
- Manages work schedules for persons supported in order to maximize availability of job coach supports for each person supported in the Supported Employment Program (SEP).
- Ensures that persons supported are meeting all requirements of the community employer.
- Assists person supported with completing community job tasks while also teaching and encouraging independence.
- Assists person supported with all communication needs.
- Ensures persons supported participate and engage in all work assignments with limited assistance to the greatest extent possible.
- Represents Pathway while on duty in the community by wearing assigned Pathway attire and employee ID badge. Communicates with customers, public and co-workers in a professional manner.
- Complies with all community employer’s workplace policies and procedures while providing supports to people supported.
- Documents all hours SEP services are provided to persons supported.
- Maintains safe and orderly workspace.
- Coordinates and prioritizes multiple job tasks and work activities in vocational areas.
- Responsible for merchandise, material, and equipment at all times.
- May be required to adjust work hours for special program needs and events.
- May have varied schedule, if needed. May be required to complete contracted hours in Community Day Services, as necessary.
- Maintains proper driver eligibility and background clearance qualifications as set forth for Agency employment.
- May be required to obtain medication administration authorization as set forth by DHS regulations. May be expected to demonstrate required Direct Support Person (DSP) knowledge of individual medications, side effects, and proper documentation processes, as needed.
- Completes all annual training requirements.
- May be required to assist during emergency situations.
- Performs all other duties as assigned.
Skills and Qualifications
Verifiable documentation of education. High School graduate (or equivalency). At least 18 years of age with valid driver’s license and insurable driving record. Must be DSP (Direct Support Person) Trained. One or more years’ experience programming for persons with intellectual and developmental disabilities and other physical and mental challenges required.
Successful completion of the Short Test of Functional Health Literacy in Adults (S-TOFHLA), testing ability to read. This is a skilled position. This job requires good verbal communication skills including the ability to read and write. Must be able to deal with problems and resolve conflicts. Must be able to work independently. Ability to interact with people supported by the Agency, employees, supervisory/management staff and members of the community. May represent the Agency at a variety of events.
Must be able to complete all physical requirements of the position with or without a reasonable accommodation. Successful completion of Essential Functioning Test (EFT) verifying ability to lift at least seventy-five (75) pounds.
