Position Details:
- Main Office Location: Westwood, MA
- Job Type: Full Time
- Schedule: Monday - Friday
Responsibilities:
- Engage, inspire, and prepare your team through daily clinical meetings, both on-site (three times a week) and remotely (two times a week).
- Review patient referrals to assess home care needs and coordinate appropriate services.
- Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing a plan of care.
- Review and approve revisions to patients' care plans efficiently through EMR workflows.
- Utilize EMR, Email, Ring Central, and other systems effectively to enhance communication with your clinical staff.
- Lead and participate in the interviewing, onboarding, and training of new hires at your center.
- Facilitate and conduct planning, implementation, and evaluation of in-services and continuing education.
- Assist in formulating, revising, implementing, and evaluating company goals and objectives, ensuring buy-in from the clinical team.
- Conduct morning briefings on-site once a week and remotely for the remaining days.
- Create and manage the on-call schedule and weekend operations.
Qualifications:
- Active license as an RN, PT or OT in MA.
- Maintain current BLS CPR certification.
- Minimum of two (2) years of direct clinical experience.
- Prior supervisory experience preferred.
- Excellent written and verbal communication skills.
- Proficient with computers, tablets, smartphones, EMR systems, email, and other electronic systems.
- Ability to foster a positive and purposeful culture.
