Pathways Nursing and Rehabilitation Center is a 112 bed complex care nursing home specializing in : all Pediatric needs, Adult Ventilators, Adult Traumatic Brain Injury and Rehabilitation. Recently, Pathways underwent a complete building renovation, and offers a pleasant and modern work environment.
Duties and Responsibilities :
Demonstrates knowledge of age specific developmental factors specific to adult, geriatric and pediatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs.
Demonstrates knowledge, skills, and techniques necessary to plan social services programs for residents with the following needs : therapeutic, neurological and dementia, dialysis, IV therapy, infectious, Ventilator and Pediatrics
Promptly follows-up on admission inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement
Other duties as assigned
Qualifications :
Experience as a Director of Social Work in a large LTC setting preferred
LMSW preferred, or Bachelors degree in related field
At least two year experience in Social Work field required
