Pathways to Housing DC (Pathways DC) is an innovative nonprofit agency committed to ending homelessness for individuals with serious mental illnesses, co-occurring substance abuse disorders and/or other medical challenges in our Nation's Capital. Pathways is one of the originators of "Housing First", in which services are provided to those in need without conditions and pre-housing requirements such as curfews, mandated sobriety or compliance with medication.
The Housing Program Manager oversees both Project Home and Housing Initiative Program (HIP) in Montgomery County, MD. Both programs provide flexible, community based services that are designed to promote the stability, recovery and community reintegration of formerly homeless individuals. This role ensures that the program deliverables, case manager development, and clients’ needs are being met by overseeing the program and reporting monthly outcomes to their direct supervisor.
RESPONSIBILITIES:
- Collaborate with the Director of Maryland Programs to create a supportive culture where both clients and staff can thrive.
- Determining staffing needs, interviewing potential employees, and oversee assigning of caseloads.
- Train and supervise 5 Case Managers
- Submit monthly reports to contract monitors and the Director
- Supervise electronic and written record-keeping activities (e.g., progress notes, service plans, client budgets, incident reports and client charts) weekly
- Collaborate with the ACT Clinical Director Outreach, Program Manager, Medical Director, and outside agencies as need for client support and best practice
- Attended all meetings related to housing programs with outside agencies. Included but not limited to: Coordinated Entry System meetings, ICH sub-committees, case transfers, HIP Supervisory meetings, ACT treatment planning, and collaborative workgroups.
- Facilitate weekly team meetings for case managers for case support and to monitor the needs of staff and clients
- Perform other related duties as assigned
QUALIFICATIONS:
- Bachelor’s Degree in related social service or healthcare related discipline; Masters Preferred. Supervision towards clinical licensure available.
- At least two years’ experience with housing voucher programs
- Supervisory Experience
- Working knowledge of social service and individual support services and also Maryland and Montgomery County community resources. Familiarity with HMIS.
- Excellent organizational and both written and oral communication skills with the ability to communicate effectively with individuals with varied cognitive abilities to establish professional relationships
- Ability to work independently
- Excellent computer skills
- Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations.
- English/Spanish strongly desired BONUS OFFERED.
- Driver’s License and access to personal vehicle required.
