- Working in a high-volume environment and dealing effectively with rapidly changing priorities.
- Utilizing a system of tools and IT platforms to support the rapidly changing needs of patients.
The Opportunity
PAF is currently seeking a Case Manager I to join our team in Hampton, VA. Case Managers actively engage to solve patient’s issues, identifying their root causes, developing a plan of action, and interacting with external stakeholders on their behalf to ensure access to care, workplace protections and preservation of their financial stability.
A Case Manager I will have expertise and knowledge of:
- Ways to address a patient’s financial concerns include cost-of-living expenses, medical expenses, and other axillary costs.
- Landlord or mortgage lender negotiations to work towards avoiding eviction or foreclosure.
- Locating and securing lodging and transportation for patients traveling for treatment.
- Mediating resolutions to utility shut off or delinquent payments.
- How to identify and help enroll in available local, regional, and national programs that offer financial assistance for cost-of-living needs.
- Address food insecurity.
- Providing critical education and help to access workplace protections including COBRA, FMLA, ADA and employer-based disability benefits.
- How to locate and link patients to available emotional support groups.
In this role, a Case Manager I will:
- Be the primary point of contact for patients with serious health conditions.
- Provide timely contact and follow up to analyze the issue(s), investigate to determine the root cause and develop a plan of action and follow through.
- Secure, complete and distribute appropriate forms to proper agencies. Conduct timely follow-up with both the agency and patient to ensure processing and communicate outcome(s).
- Provide hands on intervention, utilize conference calling for discovery, negotiate with lenders, seek out financial assistance programs, screen patients for eligibility, and facilitate/complete appropriate applications to remove financial barriers patients face with their daily cost of living expenses and auxiliary costs as related to their diagnosis.
- Creating and sending appropriate communications to patients, including follow up and closure letters, surveys and/or authorization forms, when applicable as well as supplemental educational materials.
- Entering and maintaining and accurate case related information into the program database throughout the engagement of the case
- Actively engage with patients and their families over the phone in a timely and professional manner.
Qualifications
If you have worked in one of these areas and are ready to put your skills to use in a different way, this role could be for you! The ideal candidate for a Case Manager I role with PAF will have:
- 2 years of professional background in one of the following areas: financial navigation, health care navigation, health insurance, social work, social services, charitable programs, housing counseling, government office, or utility aid programs working with the financial needs of individuals, preferably with an understanding of serious health conditions and treatment.
- Hands-on experience in one or more of the following areas:
- Helped clients seeking various forms of financial assistance for daily cost of living expenses in a local, regional, state, national or disease specific charities.
- Worked with clients with food insecurities to get needed food or food supplements.
- Worked with clients in the housing authority, other agencies or rental company helping with housing barriers to include search for affordable housing, rent delinquencies, eviction, and homelessness.
- Helped clients determine eligibility, enrollment, and follow through to application decision with the department of social services on SNAP, WIC, general aid, utility assistance, and childcare.
- Worked in the mortgage setting addressing delinquencies, forbearance, and foreclosure.
- Helped clients apply for and address utility delinquencies and shut off notices.
- Helped clients understand their employer protections such as FMLA, ADA and awareness of state and federal employment laws.
- Customer service background essential, with understanding of and commitment to providing a consistently high-level of service to patients and their families.
- Possess strong research and problem-solving skills.
- Demonstrates a high level of initiative and ability to self-direct to solve financial problems of patients, frequently working collaboratively with external entities.
- Excellent organization, time management, conflict resolution and negotiation skills required.
- Maintain confidentiality and comply with patient privacy guidelines.
Compensation and Benefits
Offers are based on relevant experience, skills, certifications and/or licensure within a healthcare environment. The offer is contingent upon favorable background and credit checks. This role qualifies for a hybrid work schedule, working 2 days a week from home and only 3 days a week at our Hampton, VA location after training and mentorship period.

Starting pay range is: $20 to $22 per/hr.
