Key Responsibilities of an MCO Team Manager in Patient Care Coordination:
This role specifically oversees the PA Health and Wellness Team internally at PCC.
Team Leadership and Management:
- Supervising and mentoring team members, including patient care coordinators or case managers.
- Assigning tasks and caseloads to team members.
- Ensuring team members adhere to MCO guidelines and protocols.
- Providing ongoing training and development opportunities for the team.
- Resolving team conflicts and addressing performance issues.
Patient Care Coordination:
- Identifying patients with complex needs or those who may benefit from care coordination.
- Developing and implementing individualized care plans that address the patient's specific needs and goals.
- Facilitating communication and collaboration among healthcare providers, patients, and families.
- Ensuring timely access to necessary medical and social services.
- Monitoring patient progress and outcomes, and making adjustments to care plans as needed.
Quality Improvement and Compliance:
- Monitoring and evaluating the quality of care provided by the team.
