Job Overview
The Patient Care Manager plays a vital role in ensuring the continuity of high-quality care to home health patients. Reporting to the Clinical Director, this position oversees an interdisciplinary team of staff to deliver patient-centered care.
Key Responsibilities
- Coordinate patient referrals and ensure timely clinician assignments for evaluation.
- Determine patient eligibility for home health benefits and oversee ongoing insurance approvals.
- Assess patient needs and develop individualized plans of care that meet medical necessity and are in accordance with physician orders.
- Oversee plan updates and revisions as necessary.
- Manage phone calls from clinicians, patients, referral sources, and communicate patient updates to relevant parties.
- Review daily assessments and plans of care, consulting clinicians as needed.
- Coordinate care with all disciplines, physicians, and other healthcare providers.
- Follow up on lab results and clinical diagnostic tests to ensure adequate physician notification and plan modifications.
- Schedule and facilitate case conferences and SOC reports.
- Assist clinicians in coordinating patient transfers and discharges.
- Process new orders and update visit frequencies as necessary.
- Document and process verbal orders received directly from physicians.
- Complete payer change documentation properly and timely.
- Review clinician visit notes weekly to ensure timely submission of documentation.
- Evaluate service delivery to ensure appropriateness of care and utilization of services, equipment, and supplies.
- Enter infections and incidents into the online Risk Management Incident Reporting System.
- Participate in QAPI program and provide direction to clinical team members.
