About the Job
This is a leadership role that requires strong leadership and communication skills. The RN Patient Care Manager will be responsible for leading a team of clinicians in providing quality support to patients and their families.
Responsibilities
- Patient Care Coordination: Receives referrals and ensures appropriate clinician and/or therapist assignments for timely patient evaluation by signing off after authorization and plotting start of care (SOC) visits.
- Clinical Oversight: Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Individualized Care: Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
- Plan of Care Development: Oversees and assures development, implementation, and updates to the individualized patient plan of care, as appropriate.
- Communication: Manages and documents phone calls from physicians, clinicians, patients, referral sources, and communicates patient updates/new orders to clinicians. Uses coordination notes to document, as needed and appropriate.
Requirements
- Certifications: Current RN licensure in state of practice, current CPR certification required.
- Education: Bachelor's degree in nursing or related field preferred.
- Experience: At least 2 years of experience in a leadership role in healthcare.
- Skills: Strong leadership and communication skills, ability to prioritize tasks and manage multiple responsibilities, knowledge of home health care policies and procedures.
