- In this exciting role, you'll provide top-notch primary and palliative care services directly to patients in the comfort of their homes, whether it's at an assisted living facility or in their own residential home. Gone are the confines of a traditional clinic setting – instead, you'll be a dynamic mobile PCP, supported by a comprehensive suite of resources.

Duties to include but not limited to:
- Conducts medical evaluations of predominantly elderly, frail, chronically ill homebound patients in their home environment.
- Documents visit via EMR *locked in 72hours.
- Develops a treatment plan in collaboration with the patient and family and their goals of care.
- Evaluates patient medication and writes/orders prescriptions as needed.
- Orders and interprets diagnostic tests relevant to patient’s needs.
- Collaborates with multidisciplinary team members.
- Facilitates the patient’s transition within and between health care settings.
- Educates and counsels patients and their families regarding health care options.
- Makes appropriate referrals to home health or hospice as needed.
- Facilitates access to appropriate supportive care services.
- Serves and protects the health care provider practice by adhering to professional standards, policies and procedures, federal, state, and local requirements.
- Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
- Seeing on average 5-6 patients per day in homes and 8-12 patients per day in facilities.
