What are the responsibilities and job description for the Intake Coordinator position at Patients First Home Care LLC?
Intake Coordinator
Assist in the coordination of patient care, under the direction of the Intake Manager.
Answer phones, receive all intake information for paraprofessional cases, and enter the appropriate information, in the computer.
Complete intake forms and appropriately route them, for admission approval. Assist in the scheduling of appropriately skilled person to fill the case, input scheduling information on the computer, and continue to assist in the coordination of the scheduling. Schedule replacement workers, as needed, and input scheduling information. Take telephone referral information. Perform other duties, as directed. Enter patient information into computer, check Medicare eligibility. Communicate with Case Managers, families, physician offices or facilities.
Level II Background check, high school graduate, experience in a Home Healthcare setting, preferred.
