Key Responsibilities:
- Deliver compassionate, patient-centered hospice care in the comfort of patients' homes.
- Perform comprehensive assessments, develop care plans, and coordinate care with interdisciplinary teams.
- Provide education and emotional support to patients and their families.
- Manage symptoms and provide end-of-life care in accordance with established protocols.
- Accurately document care and maintain up-to-date records using EMR systems.
Qualifications:
Required:
- Active RN license in Vermont.
- Minimum of 2 years of hospice case management experience.
- Proven experience delivering hospice care in home settings.
Preferred:
- Familiarity with McKesson/Netsmart EMR systems.

Pay: $2,800.00 - $2,910.00 per week
