We are expanding our program and are looking for a qualified staff for a newly created Program Coordinator position.
24/7 Residential Group Home serving 1 individual with Prader Willi Syndrome.
Qualifications:
- Bachelor’s degree in the Human Services Field with at least 2 years experience working with individuals with mental illness
- Criminal Background Clearance
- Valid PA driver’s license with clean driving record
- Successful completion of medication administration training (to be provided upon hire)
- Typical schedule is 40 hours per week depending on the needs of the site.
- Most hours will occur between 7am and 8pm and may include weekends and holidays. The selected candidate must be available in emergency/crisis situations.
- This position also includes shared on-call responsibilities which includes securing coverage for call-offs or covering the shift yourself if alternate coverage can't be secured.
- This position includes supervisory tasks and scheduled direct care shifts working the site with 20/20 split.
- Must be able to walk and exercise with individual residing in the home which can also include two- 45 minute sessions per day.
- Must possess good written and verbal communication skilks
- Must be able to work as a team member and demonstrate the ability to lead the team.
Salary/Benefits:
- $21.00 - $22.00 per hour depending on prior experience with Paid Training

General Responsibilities:Job Type: Full-time
- To assist resident in developing goals and monitoring progress on goals
- Meal Planning and Activity Schedule to ensure maximum health and weight for the individual in our care.
- Assist developing the schedule for the Direct Care Staff and updating master schedule with daily changes.
- Supervise staff and oversee residential programming with the support and supervision of the Program Specialist and Program Director.
- Oversee medication administration process. Review MAR and pill counts daily for errors, ensure all required medication is ordered and on site when needed, update historical medication records, monitor and train staff in accord with DHS guidelines, monitor OTC for expiration dates and assist with the return or disposal of unneeded medications.
- Assist in preparing meals (supervision require around food)
- Assist with grocery shopping
- Assist with upkeep of the home and vehicle
- Accompany and engage individual in community based activites
- Accompany to and schedule doctor / dental appoitments
- Complete daily, monthly and quarterly paperwork
Salary: $21.00 - $22.00 per hour based on prior experience.
