The Role You'll Play
As a Recovery Services Care Manager, you will be a vital part of our team, ensuring that clients receive the support and coordination they need as they transition between levels of care. Your role will involve facilitating smooth transitions from outpatient services to higher levels of care, assisting with discharge planning, and maintaining strong communication with inpatient facilities. You will advocate for clients, provide education on substance use, and engage with the community to promote awareness of Pawnee’s recovery services.
Your work will directly impact clients by :
- Helping them access appropriate inpatient or detox services when needed.
- Coordinating care and maintaining communication with external providers.
- Ensuring continuity of care through thoughtful discharge planning.
- Advocating for clients and connecting them to essential resources.
- Providing crisis management and education to support long-term recovery.
