*This is a year-long contract. Once the contract expires, employment will conclude. If the employee is interested in continuing with PCC, they may be offered a permanent position, provided an FTE is available at that time.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Providers excellent customer service to internal and external customers
- Supports patient-driven requests via phone call, portal message, or provider/care team request via electronic health record (EHR), engaging patients as active participants in their care
- Assists patients in scheduling and completing referrals as needed by coordinating between the patient, provider(s), and the referral source
- Decrease barriers to care, increase motivation, and foster open communication
- Contact referral sources when reports/results are outstanding, request results/reports not automatically received by PCC
- Outreach patients after emergency room visits and/or inpatient admissions to ensure timely follow-up with primary care; obtain discharge records as needed
- Assists with training and supporting staff to follow department workflows within specialty area (Health Risk Assessments [HRAs], Care Manager delegated patient cases, patient support of HEDIs/Quality measures, proper documentation within Athena & MHNConnect2.com, etc.)
- Manage electronic health record team inboxes to ensure appropriate documents are send to primary care provider, team nurse, and other members of the care team.
- Regularly attend and participate in monthly site team meetings
- Contribute to patient education materials and strategies to support care coordination
- Work with manager and team to create flow charts, workflows and document tracking process as needed
- Work with PI department to outreach patients due for care, provide patient education, and facilitate appointments as necessary
- Follow guidelines to enhance care coordination for high-need, high-risk patients, tracking of high-risk areas as needed
- Develop supportive services and tools to address common barriers to care for PCC patients; integrate these with other initiatives for health promotion/education and access to care
- Performs other duties as required and assigned
Pediatric Care Coordinator – Job Description Addenda: In addition to Care Coordinator duties outlined in Job Description, the pediatric care coordinator works closely with pediatric clinical champions to manage a variety of special programs for pediatric patients and their families. This CC primarily educates and supports parents over the phone, identifies needed resources and connects families accordingly, and, lastly, supports the PCC clinics in carrying out workflows related to these programs.
Additional Duties include:
- Conducting Health Risk Assessments for pediatric patients by speaking with their parents over the phone
- Own the Lead exposure Follow-up workflow & Athena inbox. Tracking all lab results of elevated lead levels: Providing caregiver education after confirmed lead exposure, timing and placing follow-up blood draw orders, working with the Chicago and Illinois Departments of Public Health to coordinate care for eligible children.
- Primary support person for Developmental Wellness Clinic, where children receive timely developmental assessments by pediatrician and LCSW at their own primary care clinic in pursuit of possible Autism Spectrum Disorder diagnoses; this allows families to receive scholastic and therapeutic services in a more timely fashion. CC addresses referrals to the clinic, including family outreach, facilitating a provider case review, and appointment scheduling. Co-manage the Athena inbox with clinical champions, create DWC clinic appts in Athena, support families with external referrals, therapy placements, and early intervention referrals.
- Own the Early Intervention workflow & Athena inbox. Contact caregivers after an Early Intervention referral is placed to educate about the service, submit the referral to the state, coach parents on self-advocacy and follow-up.
- Primary support person for PCC’s Reach Out and Read (RoR) program, which serves to purchase and distribute books to children at their primary care appointments at all clinics. Order books for clinics to keep inventory stocked; complete program reports necessary to national org.
- Primary support person at PCC for Illinois’ Children and Adolescent psychiatric emergency program (SASS), in collaboration with Manager of Care Management. Outreach families to schedule primary care and mental health care follow up after PCC learns of pediatric mental health emergencies.
- Primary owner of the pediatric-related Resource Guides (e.g. Childcare), ensure listings are up to date and expand content as appropriate.
- Maintain and update records for newborn genetic and hearing screening as consistent with IDPH standards, in collaboration with Perinatal Care Coordination Manager.
Qualifications
Experience/Training:
- High school diploma required.
- Medical Assistant diploma from an accredited Medical Assistant program preferred.
- Certified or Registered Medical Assistant preferred.
- Associates or Bachelor’s degree preferred.
- Previous healthcare experience preferred
- Proficient in MS Office Business Application to include: Outlook, Word, PowerPoint and Excel
Physical Demands:
- Must be able to remain in a stationary position 50% of the time.
- Must be able to move around the clinic site 50% of the time.
- Constantly operates a computer, computer printer, copy machine, and telephone.
- Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
- Must be able to transport from one site to another.
- Must be able to cover other shifts as necessary.
- Required shift rotation.
