What are the responsibilities and job description for the MDS Coordinator position at Peace Care?
The MDS Coordinator is responsible for the timely and accurate completion of the Federal and State assessment tool, the Minimum Data Sheet. Defines appropriate criteria for determining patient status and determines the level, duration, and types of patients’ rehabilitation needs. Understands the financial concepts and implications of case mix reimbursement.

- Responsibilities / Accountabilities :
- Assists and participates in the development, organization and implementation of the Philosophy, objectives, policies / procedures, job descriptions and standards of nursing Care in accordance with the goals of the center; commitment to the Mission, Vision, Values of the Center;
- Demonstrates strong assessment and patient care evaluations skills.
- Knowledgeable in OBRA regulations.
- Acts as a liaison to the inter-disciplinary team and facilitates communication between the inter-disciplinary team.
- Reviews medical records and patient care plans for completeness and compliance to State and Federal regulations and standards.
- Directs, prepares and maintains the necessary patient records, both clinical and State mandated according to required timeframes.
- Completes the following duties :
- Completes all MDS assessments within the designated time frames.
- Participates in planning and adjustment of Medicare assessment scheduling.
- Validates the entire MDS once printed and ensures documentation is present to support MDS.
- Writes CAA (Care Area Assessment) summaries.
- Assists with and teaches care planning process to nurses and other members of the team.
- Schedules completion, accuracy and timeliness of the MDS and care plan process.
- Demonstrates working knowledge and active participation in the SNF’s Quality Improvement Program.
- Demonstrates knowledge of the age specific development factors specific to adult and geriatric patients (e.g. physical, cognitive and socialization factors) in planning the delivery of care.
- Participates in the care planning and attends the care conferences.
- Participates in theQAPI program of the nursing center;
- Serves on various committees of the Center;
- Acts as a liaison for residents, patients, families, visitors and staff to resolve concerns and issues, always with the goal of customer satisfaction;
- Performs other duties as requested.
- Specific Educational / Vocational Requirements :
- RN or LPN NJ Licensure
- Experience in long term care nursing required. Experience with Resident Assessment
