Position Overview:
As a Charge LPN at Pediatric Health Choice, you will play a critical leadership role in managing the day-to-day care of our pediatric patients. You will oversee a team of LPNs and caregivers while providing direct care to children with chronic medical conditions. Your responsibilities will include clinical supervision, patient care coordination, and ensuring the highest standard of care in compliance with state regulations.

Key Responsibilities:

• Supervise and lead a team of LPNs and support staff in providing direct care to pediatric patients.

• Administer prescribed medications and treatments as directed by physicians.

• Perform assessments, document patient progress, and collaborate with the healthcare team to develop and implement care plans.

• Ensure all clinical procedures are conducted in a safe, efficient, and compliant manner.

• Communicate effectively with families, patients, and other medical professionals regarding care plans and any changes in the child’s condition.

• Assist in the training and mentoring of new LPNs and staff.

• Monitor patient vital signs, symptoms, and conditions, reporting changes to the physician as necessary.

• Maintain accurate and timely patient records and documentation.

• Oversee the care environment, ensuring it is safe and clean for both patients and staff.

Qualifications:

• Valid LPN (Licensed Practical Nurse) license in the state of [State].

• Minimum of 1-2 years of experience in a pediatric or PPEC setting.

• Prior leadership or charge nurse experience preferred.
