POSITION SUMMARY :
We are seeking a highly qualified and experienced Care Navigator Supervisor to oversee and lead a team of certified recovery specialists (CRSs). A masters degree in social work or psychology and at least 2 years of direct experience with a substance use disorder (SUD) population are required. The Care Navigator Supervisor will play a crucial role in directing the team, managing the direct care operation, and ensuring effective care navigation and coordination for individuals with substance use and co-occurring mental health issues.
The role involves managing and supervising PSGs team of certified recovery specialists (CRSs) who engage, inspire, and facilitate meaningful recovery support services. They assist and encourage clients to explore, create, and meet their own recovery goals. CRSs promote and reinforce PSGs recovery-oriented and purpose-driven culture in which each clients values are recognized, understood, respected, and integrated into services and programming. The Care Navigator Supervisor will assist and supervise CRSs as they respond to critical situations with high-touch engagement and de-escalation skills that promote a least restrictive environment for individuals experiencing difficulties or crises related to a mental health / addiction challenge.
The Care Navigator Supervisor is a member of PSGs interdisciplinary team and participates in the development of personal recovery plans with an emphasis on sustainable, long-term recovery and professional development. They report directly to the Clinical Director and assist with other clinical responsibilities such as assessment, quality assurance, outcome reporting, and program design.
Qualifications
- Master's in social work or psychology and at least 2 years of direct experience with a SUD population required; Licensed Clinical Social Worker (LCSW) preferred
- Minimum of 5 years experience in social work or related field, with a focus on substance use disorders and co-occurring mental health conditions
- Expertise in SBIRT (Screening, Brief Intervention, and Referral to Treatment) and other evidence-based practices including Motivational Interviewing
- Strong leadership and supervisory skills, with the ability to guide and support a team of certified recovery specialists
- Proven experience in care navigation, care coordination, and consultation with external providers
- Excellent communication and interpersonal skills, with the ability to collaborate effectively within a multidisciplinary team
- Demonstrated strengths in integrated behavioral health and a commitment to providing holistic care
- Ability to manage a caseload of direct care clients when needed, ensuring comprehensive and individualized care
- Demonstrates the PSG mission, purpose, and core values in everyday language and contact with internal and external stakeholders
- Assists clients with voicing their strengths, interests and goals throughout their services including : goal plans, crisis plans and other methods of expressing individual preferences
- Utilizes a comprehensive resource guide and educates members served about local community supports and resources
- Engages and communicates with a wide range of clients served, especially those who are struggling to be active in their own recovery
- Actively participates in multidisciplinary team meetings and supervisory sessions by the clinical team
- Willing and able to assist other staff as needed to maintain a safe, healing environment for all
- Travels locally to provide outreach to clients served in the community
- Must complete all provided training courses including Crisis Prevention, Mental Health First Aid, Naloxone training, and internal courses in Motivational Interviewing, Acceptance and Commitment Therapy (ACT), and PSG curriculum
