Position Summary: Embrace lived experience by being a member of the Peer Specialists Limited team that is a non-profit peer led and peer specialist driven agency. Responsible for the day-to-day operations of the Teen Outreach Program. The Coordinator - Teen Outreach Program assist General Manager in ensuring the Teen Outreach Program (TOP) is in full contract compliance, collaborating with Milwaukee County and state DHS staff, and ensure quality services provided to the people being served.

Essential Functions:

  • Participates in regularly (monthly and quarterly) calls with DHS and promptly communicate any changes to planned activities or other happenings related to TOP.
  • Assist General Manager in monitoring all DHS contract and program requirements.
  • Successfully implement the Teen Outreach Program (TOP) with middle school youth, ensuring the intended number of youths attend at least 75% of the Wyman TOP curriculum content modules. This is described below:
  • Onsite clubs must provide at least 32 weeks of programming with each session lasting a minimum of 45 minutes.
  • Clubs must cover 12 or more TOP lessons and meet a minimum of twenty-five times.
  • 20 hours of meaningful Community Service Learning are needed for each teen completing TOP.
  • Pilot clubs occurring over a shorter duration of time must be pre-approved by Wyman and DHS.
  • Agencies with Wyman and DHS approved implementation adaptation applications may implement TOP with adjustments to the program requirements outlined above.
  • Ensure all TOP clubs are led by facilitators who:
  • Have successfully completed the TOP Training of Facilitators through Wyman.
  • Are assigned to the same TOP club(s) for the duration of each club to build relationships with participants. In the case of brief absences, such as staff illnesses, substitutes are allowed and encouraged.
  • Maintain a ratio of at least one facilitator per 25 young people, ideally with a ratio of between 1:10 to 1:20.
  • Have completed an evaluation orientation with UW Population Health Institute Evaluation Team.
  • Have completed any other requirements for working with youth as required by program site.
  • Ensure all supplied content and communications are:
  • Aligned to the Positive Youth Development framework.
  • Free of bias, blame, and stigma.
  • Inclusive to the needs of all youth including Lesbian, Gay, Bisexual, Transgender, Intersex, Queer and Questioning (LGBTIQ ) youth.
  • Medically and culturally accurate, recognizing the experiences of youth from diverse communities, backgrounds, and experiences.
  • Not changed from the approved TOP curriculum.
  • If providing services in public school districts, adhere to the requirements of Wisconsin Statute 118.019 regarding human growth and development, in collaboration with the district.
  • Collaborate with General Manager to set up Project Wise sites must ensure all clubs receive the following required lessons from TOP curriculum:
  • Introduction to Reproductive Anatomy and/or Changes During Puberty
  • Sexual Health Myths and Facts
  • Talking About Abstinence
  • Collect and regularly report on data related to TOP including sequencing forms (weekly), Wyman Connect (weekly), and narrative reporting (bi-annual) as requested by DHS and Wyman. Ensure all TOP participants receive entry and exit surveys, including late enrollees.
  • Recruit and keep participants for TOP clubs. Engage parents and guardians in the consent process, using the required consent language provided by TOP. Consent can be passive or active.
  • If collaborating with community partners to provide TOP, ensure a signed Memorandum of Understanding is on file from each partner with which the organization plans to conduct a TOP Club prior to beginning the club. MOUs include roles and responsibilities for each partner, ensure adherence to data collection and program requirements, should be easy to read and understand, and fair to each party.
  • As appropriate, provide referrals to health-related services -- referral resources should include providers of health care services (e.g., substance abuse including alcohol, tobacco cessation, mental health issues, and domestic violence and sexual assault response providers). Encourage the enrollment of eligible adolescents in public aid programs such as Medicaid, Badger Care Plus, or any other federal or state aid program for which the adolescents may be eligible. Referral services on contraception may not include demonstrations, simulations, or distribution of contraceptive devices. All referrals should be documented, communicated to DHS, and reported in bi-annual narrative reports.
  • Create, support, and facilitate a youth governance council focused on advocacy to meet the needs of youth.
  • Assist in developing community collaborations to support the needs of youth being served by the program.

Other Duties and Responsibilities:

  • Attends and takes part in PSL team meetings
  • Other job-related duties as may be necessary to fulfill the responsibilities of the position.

Work Relationship and Scope:

Reports to the General Manager. Has contact with teens, young adults, and youth in 6th, 7th and 8th grades. This also includes contact with parents/primary caregivers to support youth involvement. staff, and other collateral contacts. Other contacts may include Milwaukee County Department of Health and Human Services, Milwaukee County Division of Youth and Family Services (DYFS), Children’s Court officials, MPS staff and administrators, staff of youth serving agencies and the public.

Demonstrates a strengths-based, person centered, and culturally intelligent approach in facilitating youth groups.

Working Conditions:

Work is performed in a busy office environment and in the community serving parents/caregivers, children, young adults, and families. Some of the work is done sitting at a desk using a computer; requires significant outreach in the community and families’ homes; much of the outreach is done in communities throughout Milwaukee and several hours per day may be spent driving; hours average 40 per week; flexible work schedule includes some hours outside the normal work schedule on evenings and weekends.

Physical Demands:

Position is mobile with time spent in the community, including home visits and time spent in the office; driving throughout Milwaukee County is required; must be able to go up and down stairs.   • Strong understanding of the ability to clearly communicate these to others
  • Knowledge of agency policies and procedures and the ability to relay them to others
  • Ability to be a leader during crisis situations
  • Strong supervision skills and the ability to maintain and promote positive working relationships
  • Excellent assessment and interviewing skills
  • Ability to support staff in difficult situations
  • Effective use of time and resources to help maintain quality services.

Minimal Qualifications:

  • Bachelor’s degree in social work, Human Services, or Criminal justice preferred;
  • 2 years of experience providing support to youth and young adults
  • 2 years supervisory experience preferred
  • No convictions or license revocations that would not allow to serve people based on Milwaukee County standards
  • Valid driver’s license, automobile, and sufficient insurance to meet agency requirements.
  • Complete all Wraparound trainings as required for the position
  • Ability to display cultural competence by responding respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions, and other diversity factors in a manner that recognizes, affirms, and values the worth of everyone. Knowledge and skills to work with young adults, and families.
