Job Description
Participates in the coordination, development, implementation, and evaluation of the clinic’s overall Quality Improvement (QI) program, which includes all areas of clinic and satellite site operations. Quality improvement activities encompasses both internal and external quality initiatives including an affiliation such as Accountable Care Organizations (ACO) or other contracted quality programs.
Assess, analyze, Recommend and facilitate clinic quality processes to ensure delivery of the highest quality patient continuity of care.
Identify and problem solve clinic quality with internal stakeholders (providers, managers, staff) and external stakeholders (patients, vendors, contracted organizations, grants) etc.
Identify, develop and implement data driven strategies to improve patient experience, reduce costs and improve clinical outcomes.
Assess, develop, implement and evaluate workflows impacting quality performance, quality compliance and gap closure for internal and external programs.
Develop a framework for quality measurement and process improvement across clinic sites.
Establish and maintain an audit tracking process for relevant clinic quality metrics.
Analyze quality data, establish targets/goals, make recommendations/process improvement and generate reports related to clinic quality metrics.
Participate and manage external quality programs such as ACO, PCMH and other relevant activities across clinic sites.
Provide and support staff education and development in clinic quality data and quality improvement activities.
Utilizes the nursing process, for the performance of patient care, to assess, plan intervene and evaluate patient care to achieve prioritized patient outcomes.
Assists provider during clinic in accordance with RN scope of practice, collects and prepares specimens for lab tests, maintains instruments/equipment, preps patients for procedures.
Performs other duties as assigned.
Hours: Minimum 36 hours/week, Monday-Friday
Experience and Skills
Education and/or Experience
Bachelor's prepared education preferred
RN required
Professional education in a health-related field such as nursing, health administration, etc. with at least 2 years of experience.
Working knowledge of clinical quality principles, practices, methods and tools, preferred
