Position Overview
We are seeking a dedicated and detail-oriented Patient Intake Coordinator to join our team. In this role, you will manage the intake process for new patients, ensuring all necessary information is gathered, documented, and communicated effectively to the operations and clinical team. Your organizational skills and compassionate approach will help provide a seamless experience for patients and their families.
Key Responsibilities
- Initial Contact: Serve as the initial primary point of contact for patients and families, providing information about services and answering questions regarding the intake process.
- Information Gathering: Collect comprehensive patient details, including medical history, insurance information, and specific care needs.
- Assessment: Conduct preliminary evaluations to determine the suitability of services and identify any special requirements for care.
- Coordination: Work closely with the clinical nursing team and the administrative team to ensure all necessary documentation is completed and admissions are timely.
- Documentation: Maintain accurate records in the system, ensuring compliance with regulatory and company standards.
- Communication: Effectively relay information to patients and families regarding the status of the process.
- Quality Assurance: Participate in initiatives aimed at improving the intake process and enhancing patient satisfaction.
- Renewal: Assist operations and clinical team in renewing home health care service.
Qualifications
- Education: Medical Assistant Diploma or Associate’s or bachelor’s degree in healthcare administration or Social Work or Biology is preferred.
- Experience: Minimum of one (1) year of experience in a healthcare setting, preferably in admissions, intake, or patient services.
- Bilingual: Fluency in English and Spanish is strongly preferred due to patient population needs.
- Skills:
o Strong communication, interpersonal abilities, and writing skills.
o Proficiency in using electronic health record (EHR) systems and other healthcare software.
o Critical thinking and strong analytical skills to evaluate the situation.
o Exceptional organizational and time-management skills.
o Knowledge of health insurance processes and medical terminology.
o Bilingual (English and Spanish) required to effectively communicate with patients and families.
Physical Requirements
- Ability to perform duties requiring extended periods of sitting and working on a computer.
- Willingness to travel locally for patient intake.
