What are the responsibilities and job description for the Personal Service Contract - Case Manager position at Pennyrile Area Development District?
The Pennyrile Area Agency on Aging has a contract opportunity for a case manager in Todd and Muhlenberg County. This position is a Personal Service Contract with the Pennyrile Area Development District (PADD).
PeADD is committed to recruiting and retaining individuals in a wide range of skills and occupations. We need exceptional people who share our passion for promoting, developing, and protecting the economic, health, education and quality of life of the people of our nine-county area.
Duties will include providing assessment/case management services to determine the client’s eligibility for in home services. Provide in home visits every other month. Maintain necessary documentation. Develop plan of care and ensure referrals are made for needed services.
The qualified case manager shall meet one of the following qualifications:
(i) Bachelor’s degree in a health or human services field from an accredited college or university, with:
1. One (1) year experience in health or human services; or
2. The educational or experiential equivalent in the field of aging or disabilities;
(ii) RN currently licensed as defined in KRS 314.011(10), and has at least two (2) years of experience as a professional nurse in the field of aging or disabilities; or
(iii) A Master’s degree, in a health or human services field, from an accredited college or university can substitute for the required experience.
Pre-requisite job training and personal transportation is required to carry out contract functions. Excellent communication, organizational, and computer skills are a must. This position will remain open until filled. Join PeADD and help us improve the lives in the Pennyrile region of Kentucky!
