What are the responsibilities and job description for the Children's Long-Term Support Case Manager position at Pepin County?
This position will maintain a caseload of Children’s Long-Term Support Program cases, which serves children and youth with medical/physical, developmental, emotional, and mental health needs.
DUTIES AND RESPONSIBILITIES:

· Accepts requests and referrals for agency services
· Explains scope of services and consumer rights and responsibilities in relation to the use of services
· In collaboration with parents/caregivers, children/youth, and their supports, conducts strength-based assessments through interviews, home visits and collecting of collateral information
· In collaboration with parents/caregivers, children/youth, and their supports, determines the range and type of needed services
· Coordinates appropriate services for children/youth based on their assessed needs
· In collaboration with the parents/caregivers, children/youth, and their supports and providers, assess ongoing needs to update service plans
· Prepares correspondence, reports, and records as necessary and appropriate
· Coordinates and facilitates team meetings with the goal of bringing people together to collaboratively work with and for families
· Coordinates and monitors ongoing delivery of services to consumers.
· Participates in committees and task forces as assigned
· Maintains documentation that meets professional practice, billing expectations, and program standards
· Under direction of the Social Work Manager, this position may at times be required to work overtime hours or shifts
NON-ESSENTIAL FUNCTIONS:
1. Attend local and regional meetings as instructed
2. Attend professional development trainings/conferences
3. Participate on committees and workgroups as requested
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
- Bachelor’s degree in social work or closely related human services field
- Three years’ experience with individuals or families with mental health, substance abuse, and/or abuse and neglect issues preferred
- Knowledge of child development and childhood disorders
- Good written and oral communication skills; tact and courtesy; thoroughness and dependability
- Knowledge of the functions, procedures, organization, policies, and regulations governing the administration of the Children’s Long-Term Support Program
- Knowledge of and ability to provide services to children and youth with special needs and their families
- Ability to maintain confidentiality in dealing with all pertinent information
- Ability to establish and maintain effective working relationships with peers and supervisors
- Valid motor vehicle operator’s license and/or available means of transportation required.
- Background checks required
- Social Work Certification is preferred, but not required.
Job Type: Part-time
Pay: $26.46 - $30.24 per hour
