Summary Objective
Person County Government is seeking a dedicated and qualified individual to fill the position of Public Health Nurse III for the Care Management for High Risk Pregnancies (CMHRP) and the Care Management for At Risk Children (CMARC) programs at the Health Department.
This position is responsible for providing supervision, coordination, and case management for both programs, ensuring that eligible clients in Person County receive essential services. Duties include conducting newborn and post-partum home visits to eligible families and providing dental varnishing services to qualified clients.

This is a non-exempt position working 37.5 hours weekly/1950 hours annually.
The starting salary will be determined with consideration given to both the candidate's educational background and relevant work experience.
Must possess a valid driver’s license and have an acceptable driving record per County policy.
Essential Functions
- Conduct initial and ongoing assessment of client risk factors and comprehensive medical and psychosocial needs.
- Program specific assessment and evaluation forms are used. The initial assessment aids the care manager in determining the client/family service needs, which may include psychosocial, nutritional, medical, education and financial factors. This process helps to identify services needed that will help the client improve the total family situation.
- Facilitate self-determination and self-care through the tenets of advocacy, shared decision-making, and a holistic approach.
- Promote the use of evidence-based care.
- Collaborate with medical management facilities including hospitals, primary care physicians and outpatient clinics to assure established medical homes and healthcare networks for client’s access to care.
- Identify barriers to care and work with the client and other team members to overcome access to care for each program identified population.
- Collaborate with the client and team members to implement the client-centered plan containing goals to meet client needs.
- Utilize motivational interviewing skills when attempting to engage clients.
- Prioritize face-to-face client interactions over telephone interactions by conducting visits in convenient locations such as within the client’s home, at the PMH provider office, during hospital visit or other approved location deemed safe.
- Open and close caseload based on program criteria.
- Respond to all referral sources within required timeframe.
- Assess and follow up on shared care alerts as they become available to assure receipt of appropriate preventative and primary care services.
- Utilize program guidance documents and tools to develop tasks and interventions appropriate for referral reason and client/family specific needs.
- Provide education and referrals for client to agencies and community programs for help in financial assistance and budgeting.
- Utilize self-monitoring tools to ensure that documentation meets requirements in the standardized plan.
- Document services such as the comprehensive health assessment, psychosocial screening tool, and care plan in Virtual Health for clients in active case status in accordance with the CMHRP and CMARC standardized plan and guidance/policies.
- Participate in sponsored webinars, trainings and continuing education opportunities to enhance skills, knowledge and abilities.
- Participate in Community Care of North Carolina (CCNC) and Local Health Department sponsored meetings, trainings, continuing education and mentoring opportunities to share program updates, best practices and develop standardized processes for accomplishing program goals.
- Attend all motivational interviewing seminars, classes and/or workshops per program guidelines
- Maintain electronic calendar for appointments, training and shared communication.
- Meets Agreement Addenda (AA) required components and actively prevent the need of a corrective action plan through evidence of quality reporting and performance indicators.
- Represents CMHRP, CMARC and the Health Department within the community on various committees assigned, advisory boards, and during public service events.
- Works with a variety of electronic data systems, automated office systems, and manually with office equipment per product manufacturer.
- Provide coverage for other program staff (immunization, communicable disease, rabies, laboratory, family planning, maternal health, sexually transmitted infection) in cooperation with direct supervisor and/or Health Director to ensure coverage for all clients.
- Responsible for following all agency, county, state and federal applicable policies and procedures.
- Other duties as assigned by direct Supervisor and/or Health Director.
Minimum Education and Experience Requirements
- Graduation from a four-year college or university with a B.S. in Nursing which includes a Public Health Nursing rotation and two years of Public Health nursing experience; or
- A Master's in Public Health and graduation from a school of professional nursing and two years of professional nursing experience; or
- Graduation from a school of professional nursing and three years of professional nursing experience two of which must have been in Public Health; or
- An equivalent combination of training and experience;
- Degree(s) must be received from appropriately accredited universities.
Additional Training/Experience:
- Postpartum/Newborn home visit program training
- Dental varnishing program training
Special Certifications and Licenses
- Must hold a current and valid license as a Registered Nurse in the state of North Carolina or hold a multistate license without restriction within the state of residence that allows for physical care as a Registered Nurse within North Carolina.
- Must possess a valid Driver’s License in the state of residence.
- Must have a valid CPR course completion certificate/card that requires return demonstration of skills assessed by the instructor (preference to American Heart Association Basic Life Support Healthcare Provider)
- Individuals employed in a local Health Department without a B.S. Degree which includes a Public Health Nursing rotation or a Master’s in Public Health, will be required to complete an introductory course in principles and practices of public health nursing sponsored by the North Carolina Department of Health and Human Services- Division of Public Health within one year of employment as a condition of continued employment in accordance with Health Services Commission Rule, 10 NCAC 46.0301.
