JOB SUMMARY: We are seeking a driven and relationship-focused Marketer to facilitate the transition of appropriate patients from our Licensed Home Care Services Agency (LHCSA) to our Certified Home Health Agency (CHHA). This individual will play a vital role in identifying patients receiving personal care services who may benefit from skilled home health services, ensuring a seamless referral process and continuity of care.

RESPONSIBILITIES:

Referral Development & Relationship Building:

  • Identify and cultivate opportunities to transition eligible patients from LHCSA services to CHHA for skilled nursing, therapy, and other medical services.
  • Educate LHCSA caregivers, coordinators, and families on CHHA services, eligibility requirements, and benefits.
  • Build strong relationships with LHCSA staff to encourage timely and appropriate referrals.
  • Serve as a liaison between LHCSA and CHHA teams to ensure smooth handoffs and positive patient experiences.


Patient Transition Coordination:

  • Assess patients receiving LHCSA services to determine if they qualify for CHHA services.
  • Collaborate with LHCSA coordinators and CHHA intake teams to facilitate a smooth referral and admission process.
  • Communicate with patients, families, and caregivers about the transition from personal care to skilled home health services.


Marketing & Outreach:

  • Conduct in-service trainings for LHCSA staff to ensure understanding of CHHA services and referral guidelines.
  • Develop marketing materials and referral guides tailored to LHCSA caregivers and coordinators.
  • Organize educational events and engagement initiatives to increase awareness of CHHA services.


Data Tracking & Performance Monitoring:

  • Maintain accurate records of referrals, conversions, and patient transitions.
  • Analyze referral trends and provide leadership with insights and recommendations for improvement.
  • Ensure compliance with all regulatory requirements related to patient referrals and transitions of care.


QUALIFICATIONS:

Education: Bachelor’s degree in Marketing, Healthcare Administration, Business, or a related field preferred. Equivalent work experience will be considered.

Experience:

  • Minimum of 2 years of marketing, business development, or referral coordination experience in home health, home care, or healthcare services.
  • Knowledge of LHCSA and CHHA operations, services, and eligibility requirements.


Skills:

  • Excellent relationship-building, communication, and networking abilities.
  • Strong organizational and follow-up skills to ensure successful patient transitions.
  • Ability to work collaboratively across multiple teams and departments.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) and CRM software.


Other: Valid driver’s license and ability to travel within the service area as needed.

Job Type: Full-time

Pay: $70,000.00-$80,000.00 per year
