About Us:
The Schuyler Inn is dedicated to providing safe and supportive temporary emergency housing for homeless families. We offer comprehensive services aimed at empowering individuals and families to achieve stability and self-sufficiency.
Position Summary:
We are seeking a compassionate and dedicated Case Manager to provide direct support and case management services to homeless families. The Case Manager will work with residents to assess their needs, develop individualized service plans, and connect them with essential resources, including housing, employment, healthcare, and social services.
Key Responsibilities:
- Conduct intake assessments and develop personalized service plans.
- Provide case management services, including crisis intervention and goal-setting.
- Assist families in obtaining housing, employment, education, and other essential resources.
- Advocate for clients and coordinate with external agencies for services and referrals.
- Maintain accurate and timely documentation of client progress.
- Facilitate life skills workshops and support groups.
- Collaborate with shelter staff to ensure a safe and supportive environment.
- Monitor and evaluate the effectiveness of service plans and adjust as necessary.
Qualifications:
- Associate’s degree in Social Work, Human Services, Psychology, or a related field (Bachelor’s preferred) and 1-2 years of experience in case management, social services, or working with homeless populations.
- OR high school diploma/equivalency and 5 or more years of experience in case management or working with homeless populations.
- Strong understanding of housing resources, mental health services, and government assistance programs.
- Excellent communication, problem-solving, and organizational skills.
- Ability to work with diverse populations and handle crisis situations with empathy.
- Knowledge of trauma-informed care and harm reduction approaches is a plus.
- Proficiency in case documentation and basic computer skills.
- Bilingual preferred but not required.
