Responsibilities :
- Collaborate with care teams to manage therapy coordination, benefit determination, and responses to changes in patient conditions.
- Ensure accurate documentation, medication reconciliation, and compliance with quality measures, including coding and charting.
- Schedule and participate in family conferences, facility-based meetings, and team case presentations.
- Assist in managing acute medical situations, chronic condition exacerbations, and short-term rehabilitation episodes.
