Job Summary:
We are seeking a detail-oriented and goal-oriented pharmacist to join our team. The ideal candidate will have a Bachelor's Degree in Pharmacy and one year of experience in long-term care.
Responsibilities:
- Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.
- Make recommendations for therapy changes to the physician and facility staff as appropriate.
- Maintain patient/resident database to include current and past medical history, clinical and laboratory data, and concurrent medication.
- Ensure appropriate laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
- Collaborate with the healthcare team to provide clinical and technical support as needed.
- Document all communication with physicians, nurses, and other members of the healthcare team in the medical record.
- Implement policies and procedures for safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs.
- Inform the Pharmacy Manager of medication-related errors.
- Assist facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
Qualifications:
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- One year experience in long-term care.
- Licenses/Certifications: As required by government regulations and business needs.
- Detail oriented, goal oriented, team player, oral communication.
