Position Expectations:
The Director of Patient Care Services’ overall goal is to ensure the quality of patient home care, through oversight of a large team of field nurses as well as direct case management. You will be responsible for coordinating patient care with the interdisciplinary team, patients and their families, and referring agencies.

Qualifications:
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current and valid Registered Nurse license in the state of Texas.
- Minimum 3 years of experience as a Registered Nurse; management experience required.
- Minimum 1 years’ experience in home health care.
- Current and valid Texas driver’s license.
- Reliable vehicle with valid and current proof of liability insurance.
- Current CPR certificate.
- Ability to travel locally and work in patient homes as needed.
- Strong English language skills; bilingual in Spanish, a plus.
- Provides 24 hour/day, seven (7) days/week on-call coverage.
Duties:
- Perform evaluation tasks, including vital signs and medication review and complete SOC.
- Develops a Plan of Care based on the patient diagnosis and needs.
- Dress or redress open wounds and assess progress of healing.
- Administer injections and IV’s when appropriate.
- Educate patients and families on home health care strategies and procedures.
- Make recommendations for devices or tools that might improve the patient’s quality of life.
- Coordinate with Occupational Therapists, Physical Therapists, Physicians and others involved in the patient’s care plan.
- Participates in Case Conference, in-service and meetings.
- Coordinates and establishes programs geared towards patient care.
- Helps hire and train staff as needed.
- Maintains all communication amongst field staff and office staff.
- Meet regularly with clinical and professional advisory community to provide written quarterly reports for the governing body.
- Meeting mandatory continuing education requirements.
- Promoting and educating regarding the concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
- Providing in-home patient care as needed.
