What are the responsibilities and job description for the Home Health Homemaker position at Philip Health Services?
Title: Home Health Aide
Department: Home Health
Reports To: Director of Ambulatory Services
Main Function:
Performs routine homemaking tasks to enable clients to remain in their homes. All tasks assigned to home health aides must fall within the parameters as outlines by South Dakota Department of Social Services Case Service Plan.
Responsibility: Patient Care
Responsibilty Statment: Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
Specific Performance Standards:
- Personal care, such as bath, shower, hair care, dressing and other personal tasks. Assistance in ambulation, feeding.
- Assist with medical routine as instructed on Case Service Plan.
- Cleaning house, such as dusting, vacuuming, scrubbing floors, cleaning stove and refrigerator, etc.
- Assistance in preparing meals.
- Home management duties, as designated by Home Health Director.
- Normal Range of Motion within the parameters as outlined by ASA Procedures Manual.
Responsibility: Record Keeping
Responsibility Statement: Contributes to the ongoing assessment of assigned patients utilizing recommended guidelines.
Specific Performance Standards:
- Record patients' medical information and vital signs
- Monitor, record, and report symptoms or changes in patient's conditions.
- Respects the rights, property and privacy of others; protects confidentiality and uses discretion when discussing information.
- Utilizes critical thinking in implementing the coordination of medical and nursing interventions.
- Exhibits awareness of needs for diverse customer population.
- Contributes to the individualized problem list/plan of care as evidenced by documentation.

Qualifications/Education:
- Physical Therapy pre-employment screening is required.
- Must be able to communicate effectively with providers and all levels of Philip Health Services, Inc. staff.
Experience:
- Must have complete familiarity with duties of a home health homemaker.
- Hight school graduate or equivalent
- Certified Nursing Assistant (CNA) certification preferred, not required
