JOB OVERVIEW:
Assist the RN Health Care Coordinator in performing comprehensive physical assessment, health teaching, health counseling, and provision of care supportive to restoration of life and well-being.
ROLE TYPE: Part-time, 32 hours per week
RESPONSIBILITIES:
- Assist the RN Health Care Coordinator in performing comprehensive physical assessment, health teaching, health counseling, and provision of care supportive to restoration of life and well-being.
- Assist the nurse with the completion of initial client medical history assessments to include the provision of phlebotomy services in the collection of admission blood work.
- Assist the nurse in the coordination of all client medical services external to the residential program.
- Assist the nurse with making referrals for clients needing further assessment or management of episodic and/or chronic health problems.
- Assist the nurse with follow-through for client medical appointments
- Maintain medical records for clients.
- Coordinate the client’s medical needs and significant diagnoses and treatment with the client’s primary therapist and facility psychiatrist.
- Assist with the coordination of client prescriptions with the pharmacy and mark the client's Medical Assistance number on the back of all prescriptions before sending them to the pharmacy.
- Under the direction of the nurse, follow up on health problems and health assessments on the evening shift.
- Maintain standard precautions among staff and residents.
- Be familiar with the Welcome Aboard Manual and enforce program guidelines and rules.
- Enhance, update, and remain current with educational advances within discipline and field.
REQUIREMENTS: Must be certified as a Licensed Practical Nurse (LPN)
