JOB OVERVIEW:
The Nurse-Family Partnership Nurse Supervisor and Public Health Nurse Home Visitor (NFP NS/NHV) uses principles of supervision, reflective practice, and motivational interviewing to provide comprehensive nursing services to first-time pregnant individuals and their families who are eligible for the NFP Program in Philadelphia. The NFP NS/NHV collaborates with the Senior Director of NFP to ensure the implementation of the NFP model is in line with established policies, procedures, and guidelines, while also providing oversight to a team of nurse home visitors. As a spokesperson and advocate for public health, nursing, and preventive health initiatives, the NFP NS/NHV supports family and community well-being. The aim is to ensure that all children are healthy, families thrive, communities prosper, and the cycle of poverty is broken. The NFP NS/NHV reports directly to the NFP Senior Director and works alongside other nurse supervisors, nurse home visitors, administrators, and senior staff to foster and enhance a strengths-based, client-centered culture.

Nurse Supervisor Responsibilities:
• Supervise a team of four Nurse Home Visitors and utilize the reflective process at regularly scheduled supervision sessions
• Promote clinical and professional development of the nursing staff team
• Ensure that NFP nursing staff team develops and maintains the competencies to deliver the NFP model
• Participate in the recruitment and hiring of Nurse Home Visitors
• Oversee and ensure timely and accurate data input and utilization of the NFP data system (Power BI), state data system, and local data system (Apricot)
• Pull, analyze, and interpret data reports to support and measure team performance and outcomes and shares with team and partners as appropriate
• Review documentation entered in state, local, and national databases for accuracy, uniformity, and completeness for quality output summaries used for state and federal evaluations and quality improvement efforts
• Participate in quarterly record audits and quality improvements initiatives
• Implements educational foundation in Nurse-Family Partnership program goals, theories, practice principles, rules of engagement and model elements through completion of all required NFP and lead agency education, including but not limited to Strengths and Risks (STAR) Framework, Facilitating Attuned Interactions (FAN) training, Ages and Stages Developmental and Social Emotional Assessments (ASQs), Partners in Parenting Education (PIPE), Mental Health and Intimate Partner Violence (IPV) trainings, Cultural Consciousness training, Dyadic Assessment of Naturalistic Caregiver-Child Experiences (DANCE), and the Mastery Assessment Plan (MAP) and other education as needed
• Coordinate/participate in the Community Advisory Council (CAB) meetings and the monthly PA Community of Practice meetings
• Utilize agency policies and procedures or participate in developing needed policies and procedures to guide practice
• Develop and maintain community referral sources for the NFP Program
• Coordinate and participate in regular reflective supervisions, case conferences, team meetings, NFP All Teams meetings, and quarterly staff meetings
• Facilitate professional development to the nurse home visitors through quarterly Joint Home Visits (JHV) and MAPs

Nurse Home Visitor Responsibilities:
• Provide in home visits and telehealth visits for regularly scheduled nurse-client visits
• Maintain a caseload of 12 active clients
• Develop therapeutic relationships with a diverse group of people over a long-term period
• Plan home visits in accordance with client goals and NFP outcomes and document all work appropriately and timely
• Support the achievement of client-identified goals and their family through one-on-one education, active listening, screening, and referral processes to community resources
• Support the policies, procedures, guidelines, and standards of NFP and the lead agency
• Adhere to ANA and PA nursing process and standards, code of ethics and the NFP model of home visitation
• Meet regularly with the NFP administrator for clinical reflective supervision Utilize nursing process to assess, plan, and document physical, emotional, social, and environmental needs of clients and their families as they relate to the NFP domains
• Consult and collaborate with other professionals involved in providing services to clients and their families
• Maintain confidentiality and follow HIPAA standards
• Contribute to a positive work environment that enables all staff to function at their highest potential
• Participate in at least one committee or other initiatives
• Assume other responsibilities as needed.

CREATE AND MAINTAIN NFP CULTURE:
• Maintain relationships based on trust, support and growth with team, colleagues, and community members
• Ensure that supervision of NFP nurse home visitors utilizes principles and concepts of motivational interviewing, coaching and reflective practice (modeling therapeutic relationships, change theory, self-efficacy, client centered, strength based, and solution focused approaches), and FAN
• Ensure that own reflective practice and ongoing professional and personal development needs are addressed
• Provide regular case conferences and team meetings as recommended by NFP model
• Effectively assist nurse home visitors in analyzing complex problems and cases

QUALIFICATIONS:
• Minimum of 2 years of active clinical practice experience as a Nurse Home Visitor
• Possesses automobile and uses for work related activities, has valid and current driver’s license and automobile insurance.
• Able to work on site or off site per assignments/commitments.
• Demonstrate commitment to valuing diversity and contributing to an inclusive working and learning environment

WORK CONDITIONS:
• Flexible schedule including some early evenings and weekends
• Work within the County/City of Philadelphia boundaries that include all city zip codes
• Sound physical condition and physically able to perform job in home and community environments
• Light to moderate lifting and transport of program/client materials
• Use of personal automobile or public transit
• Work assignments involve travel throughout designated area to multiple locations, including government and private agencies and partners
• Prolonged computer related exposure, as well as sitting and standing at workstations for long periods of time
• Local to regional travel or as required by NSO and funders

EDUCATION REQUIREMENT:
• Baccalaureate Degree in Nursing from Accredited University
• Current CPR/BLS certification

SALARY:
Pay Grade: 23
