Responsibilities:
- Triage all incoming phone calls for Hospice Team.
- Assist in referral management by gathering required information, updating referral tracker and working closely with the Director or Nursing.
- Assist in ordering Durable Medical Equipment.
- Work closely with all hospice team members to ensure compliance with required patient documentation.
- Responsible for keeping new admission paperwork and supplies prepared for Hospice Nurses.
- Compiles and organizes information for Interdisciplinary Team meetings,
- Complete referral/intake forms and route them appropriately for admission approval.
- Ability to work in a fast-paced environment and be flexible.
- Perform other duties as directed.
- Experience in a Hospice setting preferred.
- Quality Assurance experience preferred.
- Must have excellent computer skill - Excel, word
- Possess and orientation to serve
