Two years’ direct experience with the homeless
Justice Center approval
PROGRAM DESCRIPTION:
Supportive housing program for individuals who are experiencing street homelessness or are subway dwelling and are placed into agency independent apartments. Multidisciplinary staff will include professionals and paraprofessionals. An individual support plan is developed with the resident with goals and objective that focus on housing retention, community integration and recovery. Residents may present with an array of needs that may include mental health, substance abuse and physical disability.
PRIMARY RESPONSIBILITY:
Be an exemplary role model to workers and residents on a daily basis. Always conduct oneself in a professional manner.
TASKS & RESPONSIBILITIES:
- Coordinate and manage a caseload of approximately 15 residents.
- Provide at least 4 contacts/visits monthly (home visits/face-to-face meetings).
- Document all contacts/visits in progress notes including use of the apartment, safety concerns and need for repairs.
- Provide assessment, counseling, and linkage to coordination of services and advocacy as needed.
- Provide hands-on training and assistance with essential independent apartment living skills such as medication management, cooking, shopping, and budgeting.
- Assist residents with obtaining/maintaining entitlements.
- Develop client centered support plans with resident’s input.
- Inform the Program Director of issues endangering the well-being, safety, health, or mental status of residents.
- Maintain ongoing contact with collaterals, treatment teams and other service providers.
- Escort residents, on as an needed basis, to appointments in the community.
- Escort residents to emergency rooms when indicated which may require mandatory overtime.
- Participate in individual supervision with Program Director or designee.
- Participate in weekly staff meetings.
- Calculate each resident’s portion of the rent annually and as their income changes according to OMH guidelines.
- Assist resident in communicating with landlord and management companies.
- Assist in planning and attending social events for residents in the community.
- Update arrival and departure time in ADP timecard.
- Attend annual anti-sexual harassment training.
- Participate in trainings selected by Program Director
