About the Role
This role is responsible for providing direct nursing care, including assessment, plan of care, education, and evaluation, to patients certified as having a terminal illness who have elected the Hospice benefit.
The successful candidate will work collaboratively with other care team members and disciplines to maintain the flow of patient care and coordinate the delivery of supportive care to patients' families and/or significant others.
Key Responsibilities
- Assess patients' needs and develop individualized plans of care
- Provide education and support to patients and their families
- Collaborate with interdisciplinary teams to ensure seamless care delivery
- Coordinate referrals and resource utilization
- Document patient progress and outcomes
Requirements
- Current California RN license
- Basic Life Support certification
- Valid California Driver's license and automobile insurance
- Minimum one (1) year experience as a professional nurse
- Preferred: Medicare Certified Hospice experience and Public Health Nurse certification
