JOB SUMMARY:
The Stroke Program Coordinator is responsible and accountable for proactively coordinating and executing clinical quality activities and services associated with Pikeville Medical Center’s Stroke Program. The Stroke Program Coordinator provides oversight of concurrent and retrospective chart abstraction to meet all quality and regulatory requirements and ensures abstraction for stroke registry and Joint Commission is accurate and submitted timely. Trends and reports stroke quality metrics and facilitates performance improvement plans. Coordinates activities of the Stroke Taskforce. Leads the stroke service in achievement and ongoing regulatory compliance for Advanced Primary Stroke Certification. The coordinator is responsible for establishing effective treatment plans for selected patients, utilizing discharge planning techniques to enhance movement of patients to appropriate levels of care, and responsible for staff education compliance. The coordinator works in partnership with medical staff, nursing, and ancillary services to coordinate multidisciplinary services for the Stroke Program.
ESSENTIAL FUNCTIONS:
- Responds to all Code 100’s during working hours, evaluating the process and coordinating the care with the on-call Neurologist and or physician. Relays pertinent information to the neurologist/physician including but not limited to LKW, medications, history, etc. Communicates with patients/families and participates actively in the code 100 process, facilitating quality and timeliness of care.
- Rounds daily on stroke patients while reviewing care to enhance outcome as evidenced by best practice guidelines and patient continuity of care. Collaborates with ancillary modalities including but not limited to PT/OT/ST and others in ensuring delivery of appropriate care and treatment.
- Conducts daily stroke huddles and rounds with the neurologist on stroke patients daily.
- Obtains and maintains a superior level of knowledge of stroke care and treatment guidelines, abstraction guidelines, and regulatory requirements.
- Provides feedback and staff education on stroke care and treatment guidelines, order set usage, early recognition and response.
- Responsible for oversight of concurrent and retrospective abstraction of stroke quality data, including GWTG, JC, CMS, patient perception of care, and others as appropriate. Ensures stroke data quality and integrity. Ensures timely submission of data to regulatory and accreditation bodies.
- Leads the organization’s pursuit and ongoing regulatory compliance to maintain certification for Advanced Primary Stroke Certification from The Joint Commission in collaboration with the Standards and Interpretation department and organizational leadership.
- Aggregates, analyzes, and disseminates stroke data to support the program’s information needs to evaluate care processes and patient outcomes. Trends and reports quality metrics and facilitates implementation and evaluation of performance improvement plans.
- Responsible for the assessment, planning, implementation, and evaluation of the quality of stroke care using the ID-PDSA model and the team approach.
- Coordinates with data analysts in the presentation and reporting of stroke quality data. Attends and presents stroke data, outcomes, and opportunities for improvement to various physician and service-line committees.
- Plans appropriate review process in collaboration with patient, professionals, and support services to integrate quality outcomes as a primary objective of the organization.
- Plans, executes, and evaluates stroke community outreach and education events to promote stroke awareness and optimal outcomes, including stroke patient support group activities.
- Maintains communication and positive relationships with nursing and ancillary staff, physicians, leadership.
- Assists with the planning, development, review and revision of policies and procedures for the accreditation and certification process.
- Support other PI projects/teams or other organizational initiatives as assigned or requested.

JOB REQUIREMENTS:
Demonstrated proficiency in Microsoft Office programs including Word, Excel, and PowerPoint. Must demonstrate proficiency, or develop proficiency within 30 days of hire, with EPIC. Demonstrate ability to stay up to date with clinical outcome registries such as JC, GWTG, etc. Demonstrated ability to prioritize and manage multiple projects simultaneously. Able to use office equipment including, but not limited to telephones, computers, printers, copiers, fax machines, etc. Knowledge and application of PI tools and methods. Proficient knowledge and skills to support program specific data review, trend identification, and data interpretation. Strong interpersonal and critical thinking skills are required. Must be customer service oriented.
EDUCATION AND LICENSURE:
Registered Nurse, minimum of 3 years clinical experience in related field. Bachelor of Science in Nursing highly preferred. Current KY RN license is required.
Certification in Basic Life Support and NIHSS required. ACLS preferred. ASLS preferred.
WORKING CONDITIONS:
Works in a well-lighted business office environment the majority of the time. May be exposed to infections and contagious diseases. May be exposed to patient elements while performing daily rounds. Occasionally subjected to irregular hours.
There is a potential for being at risk of exposure to hazardous chemicals when working. Information regarding hazards and protective measures along with training on handling and storage of hazardous materials will be provided prior to initial assignment to work with hazardous chemicals, whenever the hazard changes, and on an annual basis.
