Responsibilities:
- Coordinate and oversee the completion of the Minimum Data Set (MDS) assessments for all residents in a nursing home facility.
- Ensure accurate and timely completion of MDS assessments in compliance with federal and state regulations.
- Collaborate with interdisciplinary team members to gather necessary information for MDS assessments.
- Review medical records, conduct interviews, and observe residents to gather relevant information for MDS assessments.
- Develop and implement individualized care plans based on MDS assessments.
- Monitor and update care plans as needed to ensure appropriate and effective care for residents.
- Conduct regular meetings with staff to discuss resident care plans and ensure proper implementation.
- Provide education and training to staff on MDS assessment process and documentation requirements.
- Stay up-to-date with changes in regulations, standards, and guidelines related to MDS assessments.
- Collaborate with other healthcare professionals, such as primary care physicians, specialists, and hospice providers, to coordinate care for residents.
- Participate in discharge planning for residents transitioning to other healthcare settings.
Qualifications:
- Bachelor's degree in Nursing or Health Information Management preferred.
- Valid license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
- Experience working with pediatric patients is a plus.
- Knowledge of MDS assessment process and familiarity with HEDIS standards.
- Proficiency in electronic health record systems such as AHT or PCC.
- Strong attention to detail and ability to analyze complex medical information.
- Excellent communication skills, both written and verbal.
- Ability to work collaboratively in a multidisciplinary team environment.
- Strong organizational skills and ability to prioritize tasks effectively.
