SCHEDULE: 40 hours, schedule Mon – Fri, 8am-430pm, 1 day a week remote.
Pays $23.48 per hour DOE
LOCATIONS: 3368 Washington Street, Jamaica Plain, Ma
SUMMARY OF POSITION:
The Housing Recovery Services Department of the Housing Clinical Services Department through the DPH/BSAS grant provides comprehensive, intensive case management support to chronically homeless individuals with histories of substance abuse and mental illness living in independent housing using the Housing First model promoting long-term stability in housing and the end of homelessness in Boston. HSS Case Managers work as part of an intensive case management team-serving tenants who may be experiencing untreated mental illness, active drug and alcohol addiction, and other major health challenges. HSS Case Managers use a harm-reduction, trauma - informed approach to working with participants, including the utilization of Stages of Change Theory and Motivational Interviewing techniques. HSS Case Managers are responsible for a specific caseload of tenants, and for collaborating with other HSS staff on program-wide efforts to stabilize and promote the individual growth of all HSS tenants. Additional responsibilities include, but are not limited to assisting tenants with all required program documentation, assisting tenants with obtaining benefits, healthcare, educational opportunities, employment opportunities, and legal aid services. Also, supporting the development of tenants' activities of daily living skills and creating housing service plans with tenants. Maintaining all necessary documentation; monitoring and reporting maintenance needs in apartments, and supporting timely program fee collection from tenants in collaboration with designated housing authority.

Requirements:
EDUCATION/TRAINING:
- High School diploma or GED
- Strong written communication skills
- Computer Proficiency in Microsoft Office Products
- Valid Mass Driver's License with a minimum of two (2) years of driving experience, and excellent driving record
- Undergraduate Degree in a related field (i.e. social work, psychology, education, nursing)
- Bi-lingual - Spanish/English
- Strong commitment to learning and using harm-reduction and motivational interviewing skills in order to promote improved behavioral health among tenants.
- Ability to set limits fairly and consistently while maintaining appropriate boundaries.
- Knowledge of effective crisis intervention techniques and ability to respond appropriately in crisis situations.
- Excellent organizational, communication, and interpersonal skills; the ability to balance many competing demands.
- Experience working as part of a team.
- Experience working with chronically homeless individuals in Housing First programs
- Experience working with individuals experiencing substance abuse, mental illness, trauma, homelessness and/or other complex disorders. Knowledge of local care-giving and advocacy resources for people experiencing substance abuse and dependence, mental illness, trauma histories, head injury, medical illness, physical disabilities, relationship violence, criminal and civil justice system involvement.
