This program works with individuals and their families within the greater Lehigh Valley community to address problems which are preventing them from achieving their long-term goals. Issues which are commonly addressed by our team include housing insecurity, school truancy, parenting support, social service system navigation, food insecurity and many more.

WHAT You'll do as a Wraparound Facilitator:
- This programs service delivery model is an evidence-based practice, as such it requires that the incumbent in the role participate actively in an ongoing training and quality assurance process to ensure the highest level of customer care possible.
- Provides leadership role in taking the initiative in engaging family, youth, and service providers in the HFW process with modeling effective communication.
- Facilitates team meetings with youth, family members, professionals, stake holders. Works to ensure that these meetings are conducted within the framework of the evidence-based practice service delivery model.
- Oversees the configuration and coordination of the development of teams for youth and family. Orients all stakeholders in the HFW process and maintains relationships for effective team building with cultural competency.
- Demonstrates appropriate safe work practices including crisis intervention skills. Appropriately responds to safety concerns, implements safety/crisis planning and problem solving.
- Maintains and follows through with all required documentation in a professional and timely manner. Ensures the files of each client are complete with all necessary documentation and HIPPA compliant.
- Participates in a rotating on-call system for the support of the youth and families. Ensures the wraparound process is implemented with model fidelity through the direction of the coach and supervisor.
QUALIFICATIONS:
- This position requires a bachelor’s degree in psychology, social work or a related human services field, and a minimum of two full-time years’ experience working in relevant family, children/youth or community service capacity.
- The incumbent must demonstrate strong communication/interpersonal and organizational skills.
- Must possess a broad base of life experience and a diverse view of what families need to have better lives.
- This position requires working flexible hours, and the incumbent must have a valid driver’s license and insurance, excellent driving record, and reliable transportation.
- Must demonstrate sensitivity to the cultural and socio-economic differences present among the Agency’s defined-service population.
