POSITION SUMMARY

The Health Services Specialist is responsible for coordinating client medical needs, reviewing all state mandated doctor’s visits, setting up appointments, documentation of appointments, accompany/transport clients to appointments and actively advocate for the client at the appointment, and follow-up. Ensures that appropriate insurance/medical entitlement is available and searches for medical providers as necessary.

Essential Functions Statement(s)

  • Make essential medical appointments for clients and prepare the required paperwork for those appointments.
  • Use of an organized, shared calendar for all medical appointments
  • Assist with picking up medications and communicating with pharmacy.
  • Prepare required documents for guardians to enable state-mandated medical forms to be completed by physicians.
  • Coordinate and communication with all essential staff and guardians regarding upcoming appointments.
  • Contact and collaborate with client’s house staff/DSP’s, house supervisor, QIDP, behaviorist, and residential Leadership team to gather information and provide accurate information for planning and during doctor appointments.
  • Accompany/transport clients to appointments and actively advocate for the client at the appointment.
  • Complete objective and concise medical appointment notes after every appointment, upload to EHR (Electronic Health Record) and email to appropriate parties.
  • Update client, case manager/QIDP, House Supervisors, nursing team, group home staff, family, and guardians of results after doctor appointments.
  • Explain doctor’s orders to clients and communicate any new medication changes or orders to the RN's for follow through.
  • When medication changes occur, especially psychotropic medications, contact and provide information to the QIDP for HRC submission.
  • Contact and update residential co-managers and IDD directors when there are health status changes or emergency situations.
  • Keep track of annual dental, physical, vision, hearing, 3 month seizure reviews, 3 month psych reviews, and other related exams to ensure compliance with BQM/DHS regulations.
  • Keep client medical files updated, including but not limited to: review of MAR paperwork on a monthly basis, check in of new and monthly medications with the RN and tracking of client hospitalizations, ER visits, and medical appointments on Health Summary.
  • In collaboration with QIDP and/or House Supervisor, review benefits such as insurance to ensure accuracy and update if needed.
  • Assist in monthly medication check-in and reconciliation.
  • Assist in the coverage of sick house when clients are home ill as needed.
  • Maintain DSP certification.
  • Assist in the coordination of on-site doctors’ clinics and once-a-month lab draws.
  • Regular, reliable, predictable attendance.
  • Perform all other duties as assigned by RN Supervisor, Director of IDD Residential Services, or Residential Co-Managers.

SKILLS & ABILITIES

Education: High School Graduate or General Education Degree (GED): Required

Associate's Degree (two year college or technical school): Preferred

Experience: 1 plus years of experience in an environment working with individuals with IDD

Computer Skills: Evolv, Microsoft Office, Word, Excel, PowerPoint, Outlook

Certifications &

Licenses: DSP certification within 120 days of hire or prior certification

Bus/Van training within 120 days of hire

Other Requirements: Must have and maintain a valid driver's license.

Must be able to meet agency driver safety requirement of carrying personal vehicle insurance with the following limits:

$50,000 bodily injury, each person

$100,000 bodily injury, each accident

$50,000 property damage

Ability to use Phone, Fax, Copier, and file required.

WORK ENVIRONMENT

Overall general office environment, however, a significant amount of time can be spent in an automobile during client transport. Temperatures and driving conditions can be dependent on weather. Frequent interaction with any stakeholder within the agency (i.e. supervisors, co-workers, clients, general public, outside agencies, etc.).
