Description
Case Manager’s primary focus is to address the specialized case management needs of participants in a homeless youth outreach program. The Case Manager works in coordination with partner agencies to address the broad array of client needs through referral to providers of necessary services, and in providing assistance with benefit and identification assistance. The Case Manager acts primarily as a care coordinator and client case manager, and may perform duties that include outreach, prevention and risk reduction, crisis intervention, health education, referral for substance abuse and mental health counseling, and benefits counseling. Emphasis on transitional living clientele.

ESSENTIAL DUTIES, RESPONSIBILITIES & EXPECTATIONS:
- Assess client readiness for case management services based on needs and ability and willingness to participate in program.
- Create case plans for clients with short, medium, and long-term goals and the steps to achieve those goals.
- Assist in developing savings and budget plans in preparation for eventual independent living, including the development of benefit enrollment applications.
- Conduct crisis intervention assessment with potential program youth.
- Advocate for clients with agencies and service providers to minimize barriers to clients receiving needed services.
- Follow up with referrals to ensure that clients receive and access necessary services in a timely manner
- Conduct intakes to assess housing needs, education, vocational/educational, medical, and financial barriers.
- Develop creative strategies with youth and provide resources in the areas of education, vocation, employment, social supports, finance, wellness, and housing.
- Build and maintain relationships with program stakeholders including schools and community providers.
- Represent Pivot at schools, meetings, and community events.
- Plan, implement, monitor, and evaluate youth's goals and objectives.
- Collect, evaluate, and enter data pertaining to youth’s goals and objectives.
- Follow Pivot policy & procedures.
- Perform other duties as assigned.
- Coordinate and develop transitional plans for youth graduating from high school and entering adulthood.
- Serve youth per guidelines in the Transitional Living Program
- Assist Pivot staff in presentations on behalf of the agency to advocate for disconnected and unaccompanied youth.
- Develop strategies to increase client resources & program effectiveness.
- Serve as an active liaison for referrals and school personnel.
- Develop service delivery plans for clients.
- Provide intensive level Case Management services.
- Will be active in creating pathways for referral generation.
- Carry an active caseload.
- Provide community education groups to youth.
- Assist with housing navigation
Qualifications
- EDUCATION/ EXPERIENCE
- possess a 4-year bachelor’s degree from an accredited college or university, in social services or related field, with at least two years’ experience working with youth in need
- certification as a Case Manager II or III required, if not case management certified must be willing to complete.

- OTHER QUALIFICATIONS
Applicant must:

- be at least 21 years of age, pass an OSBI criminal background investigation and not be listed in any Registry of Sexual Offenders (through fingerprinting paid for by Pivot, Inc.)
- pass a drug test (paid for by Pivot, Inc.)
- be compassionate, respectful and tolerant of a wide variety of personalities and persons including youth, parents, other team members, and those who refer youth to the shelter.

- CERTIFICATES, LICENSES & REGISTRATIONS
Applicant must:

- possess Case Management I or III certification or be working toward certification.
- possess and produce a valid Oklahoma Driver’s License and current vehicle insurance verification.
- must obtain and maintain CPR and First Aid Certifications
Salary : $19 - $22
