What are the responsibilities and job description for the Community Navigator position at Places for People, Inc?
General Description
The Outpatient Mental Health Community Navigator will collaborate with both youth and adult treatment teams to identify adult clients (persons served), caregivers, or youth case management and wrap-around care needs. The Community Navigator will then work directly with adult persons served, caregivers, or youth to plan, obtain and sustain their needed services and supports to improve and enhance their level of functioning.

Job duties include but are not limited to:
- Attend both youth and adult weekly treatment team meetings.
- Consult with therapists on a regular basis to identify needs outside of the therapeutic process completing the Community Navigator Referral Form.
- Participate in supervision with Clinical Supervisor on a weekly basis to review caseload and referral forms clarifying applicable person served case management care needs and if the focus of the approach will be for them, with them or by them with an emphasis on skill building.
- Assist with referring, scheduling, and where applicable attending service care and support needs.
- Connect and enroll in available insurance options when applicable i.e. Medicaid, Medicare, state MCOs, Affordable Care Act Plans, Gateway to Better Health etc.
- Determine additional benefits the person served may need and support to obtain i.e. Food Stamps, TANF, WIC etc.
- Resolve any identified barriers to therapeutic engagement i.e. housing, transportation, child care, technology, prescription assistance, self-help etc.
- Engage person served around an appropriate plan of action.
- Ensure the objectives and interventions related to the plan of action are captured in the person served treatment plan.
- Document all services provided
- Completes annual training requirements as outlined by agency requirements and supervisor
- Performs other related tasks as assigned by the supervisor
