General Description
The ITCD Specialist provides therapy and case management services directly to clients within the community and/or provides linkage or referral to appropriate community resources. This position also provides support and education to clients on mental health, substance use (using CRA (community reinforcement approach)), physical health, and/or infectious disease management, as well as provides advocacy and crisis intervention as needed. This is a community-based position.

Job responsibilities include but are not limited to:
- Collaboratively with clients write, implement, and monitor Individualized Treatment Plans with clients within due dates outlined per Medicaid and DMH
- Monitor clients’ medical, psychiatric and substance abuse treatment to assure effective coordination and continuity of care
- Collaboratively with clients complete psychosocial clinical assessments
- Provide services after hours based on assigned coverage as outlined by supervisor
- Facilitate client self-sufficiency and maximize independence in the community
- Provide regular visits with clients in the community
- Provide person-centered services to help clients achieve expressed goals
- Provide crisis intervention services as needed and participate in the on-call rotation
- Complete all Outcomes as scheduled
- Maintain fidelity to ITCD treatment model and learn ITCD and CRA interventions to support and reinforce skills developed in ITCD and CRA therapy
- Learn and provide other group and individual therapies appropriate for the team’s clientele
- Maintain high quality and achievement of targeted results through the use of performance management and the principles and practice of CQI and meet established productivity standards
- Provide and/or arrange transportation as clinically warranted
- Maintain client confidentially as outlined by HIPPA
- Identify and analyze diverse issues impacting clients’ abilities to function optimally; implement solutions or coordinate with other resources to resolve problems
- Participate as a member of the treatment team
- Attend all pertinent grant and agency meetings
- Complete annual training requirements as outlined in agency policy
- Other duties as assigned by the supervisor
