About the organization: Plan A Health, Inc. is a nonprofit organization focused on improving access to healthcare in the Mississippi Delta. Our newest program, , focuses on improving the health and wellness of parents, caregivers, and families by providing access to free healthcare, care coordination, education classes, support groups, and more to our communities.
About the role: This is a full-time, non-exempt position that provides in-person and remote care coordination, support, health education, and resources. The Care Coordinator/Case Manager will play a pivotal role in delivering services that empower pregnant women and parenting families with children under 2. Case management services consist of completing assessments, providing education and support to families from pregnancy through the toddler stage, consistent communication and monitoring of participants, and linkage to beneficial supportive services. The position is currently funded through March - we expect that at that point it will transition to a permanent role.
This role is primarily 9-5 M-F but may require 1-2 weekend days monthly and flexibility on hours (for example, for education sessions later in the day). Any time outside of traditional working hours will be scheduled well in advance and in consultation with the employee. Employees are able to work from home in addition to their work directly in the community (home visits, education classes, etc). One day will be based in our office in Louise (Humphreys county).
Key Responsibilities include but are not limited to:
- Serve as a liaison between healthcare providers and community members to facilitate access to health services.
- Provide case management services, including assessments and referrals, through virtual and in-person home visits in one of the following services areas: Sunflower, Washington, Bolivar, Leflore, Humphreys, Holmes, Sharkey, and Issaquena counties.
- Assist families in identifying their health and social service goals.
- Support parent-child engagement and foster family well-being through reflective strategies.
- Collaborate with community partners to ensure comprehensive resources and support for families.
- Conduct health promotion activities and deliver evidence-based parenting education.
Requirements
- Must be a resident of the Mississippi Delta
- Must possess a working knowledge of the Mississippi Delta and the local community, including familiarity with local history, population demographics, organizations, agencies, region, and systems.
- Must possess a valid driver’s license, vehicle, and clean driving record
- Degree in Human Services, Social Work, Public Health, or related field of study.
- Experience with issues related to community health access and service delivery.
- Experience working with women, children, and/or families.
- Possess strong interpersonal skills for effective collaboration with diverse groups
- Flexibility is key- be ready to adapt your schedule to meet family needs.
- Ability to walk up and down stairs into the clinic and lift up to 30 lb.
- Willing to travel throughout all counties in our service area.
- Preferred: Experience providing care in a family or women’s health setting
- Preferred: Previous experience in health education or community outreach
- Bilingual (English and Spanish) written and verbal communication skills not required, but welcomed.
Compensation
Pay Range: $42,000-50,000per year/ commensurate with experience.
