POSITION SUMMARY: The Clinician II collaborates with healthcare professionals to ensure a seamless care continuum for patients in a clinic setting, focusing on safety and high-quality standards. Responsibilities include coordinating patient wellness and chronic illness activities, with success measured by patient outcome performance metrics.

Minimum Qualifications

• EDUCATION: RN required; BSN preferred.
• CERTIFICATION: NM RN license and BLS certification required.
• SKILLS: Effective communication, teaching techniques, and coordination of educational materials.
• EXPERIENCE: One year in a clinical setting preferred.
  
  

NATURE OF SUPERVISION: Reports to CSVCG Dyad, supervises medical assistants, and ensures clinical staff competency.

ENVIRONMENT: Requires physical stamina and exposure to clinic conditions.
